STATLine Authorization Request Form

To sign up for Web STATLine please complete, print, and sign this form.  Before proceeding please read the account information disclosure; which more fully explains the use, capabilities, and liabilities of the system.  See our on-line policy for questions regarding the security and privacy of our site.

Please note:

• You must be a Healthcare Federal Credit Union member before applying for STATLine-Online access.
• All requests must be made in writing, using this form. ]
• You may mail or deliver the original to our office; faxes and e-mails are not accepted.

Activation

• Your account will be activated at approximately noon on the first business day following the day we receive your request.

Account Number

• Use your four-digit member number.

Password/Personal Identification Number (PIN)

• Access to your member account will be controlled by means of a password/PIN.  This password may be any combination of alphanumeric characters that you wish.  If you are already a user of our telephone STATLine your Web STATLine will be set-up with the same four-digit PIN.  Otherwise, your account will be set-up with a temporary password – please change this to a password of your choosing as soon as possible.
